Qualifying Life Events: What Counts and What You Need to Upload

A Qualifying Life Event (QLE) allows you to change benefits outside Open Enrollment.

Common qualifying events

• Marriage or divorce
• Birth, adoption, or placement for adoption
• Loss or gain of other coverage (spouse/partner job change)
• Change in residence that affects plan eligibility

Timing matters

Most QLE changes must be submitted within 30 days of the event date (some plans may differ). Late submissions may be denied by the carrier.

What documentation is typically required

• Marriage: marriage certificate
• Birth/adoption: birth certificate, hospital record, or adoption placement documentation
• Loss of coverage: letter from prior carrier/employer showing end date

How to submit

1. In Paylocity: Benefits > Life Event (or similar menu label).
2. Select the event type and enter the event date.
3. Upload documentation and submit.

Tip: Do not email sensitive documents. Use the secure upload flow in the ticket form if Paylocity upload is unavailable.
